Travel Visions Aloha for the Blind (TVA) — General & Run Club Liability Waiver

I, the undersigned, acknowledge that Travel Visions Aloha for the Blind (TVA) organizes various activities and programs for the blind and visually impaired community in Oʻahu. These may include, but are not limited to, hiking, walking, running, beach outings, fitness activities, and other recreational or community events.

I understand that participation, volunteering, or attendance at TVA activities involves certain inherent risks, including the possibility of injury, disability, or death. I voluntarily assume all such risks, known and unknown, whether arising from my own actions or those of others, including TVA staff, volunteers, or participants.

In consideration for being permitted to participate in, volunteer for, or attend TVA activities, I hereby release, waive, and hold harmless Travel Visions Aloha for the Blind (TVA), its officers, members, volunteers, agents, sponsors, and affiliated organizations from any and all liability, claims, demands, actions, or causes of action whatsoever, arising out of or related to any loss, damage, or injury that may occur during or as a result of TVA's activities.

I understand that TVA strives to maintain a safe and supportive environment and often assigns sighted guide volunteers to assist participants on a 1:1 basis when applicable. However, I recognize that accidents and unforeseen events may still occur and that TVA and its volunteers are not liable for such incidents.

For activities involving physical exertion, including but not limited to the TVA Run Club, I affirm that I am physically able to participate and have disclosed any relevant medical conditions. I agree to notify TVA staff or volunteers of any health changes that may affect my ability to safely engage in activities and to bring any necessary medical equipment, medication, or assistive devices to each event.

By signing this Annual Liability Waiver and Agreement, I affirm that I have read, understood, and voluntarily accept these terms. This waiver shall remain in effect for all TVA events for one calendar year from the date of signature.
