Media Release

Media Release: I, the undersigned, hereby grant Travel Visions Aloha for the Blind (TVA) permission to use my name, likeness, image, voice, and/or video recording in connection with TVA's promotional and marketing materials, including but not limited to its website and social media accounts, for a period of one (1) year from the date of signing this waiver.

I understand and agree to the following terms and conditions:

  1. Consent: I voluntarily consent to the use of my name, likeness, image, voice, and/or video recording in TVA promotional materials.
  2. Release: I release TVA and its representatives from any liability for any claims, demands, or causes of action arising out of the use of my name, likeness, image, voice, and/or video recording.
  3. Use: I understand that TVA may use these materials for promotional and marketing purposes, including but not limited to TVA's website, social media accounts, brochures, newsletters, and presentations.
  4. Duration: This consent shall remain in effect for a period of one (1) year from the date of signing this form, after which it will automatically expire. TVA may not use my materials beyond this duration without obtaining additional written consent.
  5. No Compensation: I acknowledge that I will not receive any compensation for the use of my name, likeness, image, voice, and/or video recording.
